One of the major functions of the Personnel Division is to develop job specifications and recommendations for new job classifications. This series contains Position Classification Questionnaire Forms (Form 40) developed by the State Personnel Department for every state agency to evaluate its job classifications and to make recommendations for new job classifications. Information available on the form includes employee's name, title of position, department, division, section, place of work, work hours, supervisor's name, work performed, instructions received, outside contacts, equipment used, and other benefits received. The information is then verified and supplemented by the employee's supervisors and department head. The record copy of the completed form is sent to the State Personnel Department while a duplicate is maintained by the ABC Board. In the event an employee is separated from the agency, a new form should be completed for his successor.
